Does anyone know of a uxtheme patch that works with

Windows Vista Home Premium SP2 64bit running on an AMD processor...

I tried the above website, but it messed up my Vista and I could not boot into it; luckily I have Ubuntu installed on this computer so I was able to repair the files..

Try vistaglazz

It can patch the uxtheme and if you want can make maximised windows remain transparent.

It now supports SP2 so I see no reason why it shouldn't work.

While you should always back things up, patching uxtheme.dll is done by all sorts of people all over the world and it usually causes no problem. :)
